The Risk Assessment Task guides users through evaluating and documenting organizational risks in a structured, efficient way. It outlines the steps needed to review assigned risks, complete assessment details, and record any required follow-up actions. This ensures risks are properly assessed, prioritized, and managed to support ongoing compliance.
Follow the steps below to complete a Risk Assessment Task:

You can access all tasks by clicking Show All or Go To My Tasks at the bottom of your task list in the workspace, or by selecting My Tasks from the profile menu.

Select the Risk Assessment type task from the My Assignments page. A Risk Assessment Task window will be displayed.

Note: The fields listed below are included in the templates by default. Additionally, custom fields may be added to your templates that are not included in this article. See our Template Library article for more information.

- Review the task header for the Compliance Category and Due Date.
- Add your findings in the Findings field using the formatting toolbar as needed.
- Select a Risk Decision: Accept, Mitigate, Postpone, or Resolved.

If you choose Mitigate, click Add Mitigating Actions and enter one or more mitigation items.

- Enter the required Action Plan describing steps to address the risk.
- Add your Reasoning to explain the decision and rationale.
- Use Staff Involvement to describe roles or actions expected from staff.
- Click Person(s) Involved to add the names of individuals related to the assessment.
- Toggle Notified Legal and Notified Stakeholders on if those parties need to be informed.
- Fill any additional fields shown (for example: Plano, Number Field One, B DATY using the date picker, and FAM from the dropdown).
- Check any required acknowledgement boxes.
- Review all entries to ensure accuracy and completeness.
If the Define Risk Rating option is available, only Administrators and Risk Assessment Managers can use it to define risk severity using the Severity vs Probability model. Selecting Risk Rating Key Terms (shown in blue) opens definitions for each risk level. Once both Severity and Probability are selected, the system automatically calculates and displays the Risk Rating.
- Click Save and Exit to save progress and complete the task later.
- Click Save and Complete to finalize and submit the Risk Assessment Task.
